- 1). Contact a disability lawyer. The attorney helps to develop the case, increasing chances the claim will be granted. Find Nebraska disability lawyers online, in the local phone directory or through references provided by friends and family.
- 2). Visit the United States Social Security Administration website. On the home page, under "Top Services," select "Apply For Benefits." Under "Disability Benefits," click "For An Adult" to visit the disability benefits application homepage.
- 3). Click the Step 1 link on the disability benefits application homepage to review documentation requirements. Information from form DD-214 and social security numbers for the applicant, spouse and minor children are required. The form requires income information from the prior year's W-2. Self-employed applicants enter information from the prior year's 1040 form and Schedules C & SE. Submit bank account and routing information for direct deposit of benefits.
- 4). Click the Step 2 link on the disability benefits application homepage to start the disability benefits application. Indicate if the application is being completed by someone other than the applicant. Applicants must indicate if they are visually impaired. Read the Privacy Act Statement. Mark the corresponding check box and select "Apply For Benefits." Follow the prompts to complete the application.
- 5). Click on Step 3 link at the bottom of the disability benefits application homepage to complete the disability report. The contact information of a person who knows about the medical condition in detail is required. Information about treatment, tests or prescription medication relating to the medical condition is needed. The form asks about the last 5 positions held and workers comp or insurance claims.
- 6). Read the Privacy Act Notice. Select "Start Report." Follow the prompts to complete the document.
- 7). Click on the Step 4 link located at the bottom of the disability benefits application homepage. The Authorization To Disclose Information To The Social Security Administration form is automatically loaded in a browser window. Adobe Acrobat Reader is required: Download the software for free at the Adobe website, if necessary. Print and complete the form in ink.
- 8). Send the form to a local Social Security office. Find mailing addresses on the Social Security website or in your local phone directory. The form can also be dropped off in person. Wait 3 to 5 months to hear if disability benefits have been granted.
- 9). Contact your disability lawyer to file an appeal if the claim is denied.
